Why are organizations still losing data?

Every time I see a headline about another organization losing customer data the only question I have is: how?

Seriously.

How in this day and age with all the technology we possess are companies still losing data and tapes? B&L offers RFID tape management products  that allow every tape moved to be tracked by radio frequency identification. This virtually eliminates any loss of backup media, tapes, cases or anything else our RFID tags are applied to. We’ve even written a whitepaper on developing a media management plan in networked environments .

Another alarming statistic that Beth Pariseau point outs in her blog  is that companies are only required to report the loss of information on tapes or media that is not encrypted. So you can imagine that there are probably quite a few encrypted pieces of media that are lost and never reported.
